Summary of Care Records (SCR):

Once your account has been created, you will need to provide your Summary of Care Records or private medical records showing your diagnosed condition and at least two prescribed medicines or therapies, dated within the last three months. View a sample SCR here.

Please visit our How to Obtain Your SCR help page for step-by-step instructions. Alternatively, please email your Summary of Care Records (SCR) to soc@lyphe.com when you obtain them.

Alternatively, if you’d like Lyphe to obtain your SCR for you, we can for an additional fee. If you’d like Lyphe to obtain your SCR for you, please select the £70 option in your account when booking your new appointment, instead of the £49 new appointment option.
