If you need formal proof of your medical cannabis treatment (for example for travel, legal, or insurance purposes), we can provide you with a Treatment Verification Letter. Please complete the form below and allow 48-72 hours for us to process your request and upload it to your Patient Portal.

What is included in the Proof of Treatment

  • Your name, date of birth, and patient ID.
  • Confirmation of your diagnosis / condition treated with medical cannabis.
  • The start date of treatment and current prescribed medication details (excluding dose if confidential or sensitive).
  • Signature and GMC number of the prescribing clinician.
  • Date the letter was generated.

Important Notes & Limitations

  • The letter will be uploaded to your Patient Portal once ready.
  • We provide proof of treatment letters only for active or recently active patients under our care.
  • We cannot guarantee that the letter will meet the requirements of all external organisations (e.g., customs, airlines, embassies). Please check with the relevant organisation beforehand.
  • We are not responsible for any consequences if the letter is not accepted by the third party.

Processing Time & Cost

Requests are processed within 48-72 hours for most cases. If your request is urgent, please contact us via email or phone and we’ll do our best to prioritise.

There is no additional cost for treatment verification letters for Lyphe subscription members. For non-subscription patients, a fee of £25 may apply. This covers administrative time and clinician review.

How to Request Proof of Treatment

To request your proof of treatment letter:

  1. Fill out the form above with your full name, patient ID, and specify what you need the letter for (e.g., embassy, insurance, travel).
  2. We check that you’re an active patient and verify your treatment details.
  3. Once approved, the letter is uploaded to your Patient Portal and also emailed to the address we have on file.

Frequently Asked Questions (FAQs)

What should I check before submitting my request

Ensure your patient details (name, date of birth, address) are up to date in the Patient Portal. Also, check whether the organisation needing the proof has specific requirements (e.g., signature, clinician details, letterhead).

Can this letter be used for travel or customs?

Sometimes, many embassies or airlines accept medical proof letters, but regulations vary. We strongly recommend checking with the relevant airline, embassy, or customs authority in advance. Lyphe Clinic cannot be held responsible if your medication is stopped or confiscated.

What if I’m no longer active or my treatment ended?

If your treatment with us ended recently, we may still be able to provide a letter depending on how recent the treatment was. If it’s been a long time or your case was closed, we may not be able to provide proof.

Do I need a clinician’s signature?

Yes. Proof of treatment letters always include the prescribing clinician’s signature and GMC number to ensure validity.
